Terms & Conditions
At Function & Rhythm, we strive to provide high-quality, individualized, and group-based occupational therapy services that blend occupational therapy principles and techniques with rhythmic movement to improve function. To ensure consistency, fairness, and respect for both our clients and providers, please review our policies and procedures below.
General Policies
- All participants must complete an intake form and/or occupational therapy evaluation (when required) prior to beginning services.
- Parents/caregivers must remain accessible during pediatric sessions (on-site, depending on service delivery).
- Function & Rhythm reserves the right to discontinue services if policies are not followed or if participation is deemed unsafe.
Scheduling & Attendance
- Sessions are offered by appointment only.
- Consistent attendance is encouraged to maximize therapeutic progress.
- Clients arriving late may not be guaranteed their full scheduled session time, though full payment will still be required.
Group Classes:
- Pre-registration is required.
- Full payment is required prior to attending class.
- If you need to cancel or reschedule class, please send an email and/or text 48 hours prior to class.
- Sessions cancelled with more than 48 hours’ notice may be rescheduled, subject to availability.
- No refunds are given for cancellations made with less than 48 hours (2 days) prior to the class.
- Classes may be canceled due to inclement weather, instructor illness, or other reasons beyond the instructor's control. In these cases, classes will be reschedule.
- For children's classes, a guardian or caregiver must remain accessible (within the premises or with the child during class).
- Clients arriving late may not be guaranteed their full scheduled session time, though full payment will still be required.
1-on-1 Sessions:
- An initial evaluation and plan-of-care must be completed prior to establishing recurring 1-on-1 sessions to more effectively address the client's needs.
- Full payment is required when attending each session.
- If you need to cancel or reschedule a session, please send an email and/or text 48 hours prior to that session.
- If a session is cancelled in less than 48 hours (2 days) prior to the session, a $50 late cancellation fee will be applied.
- Sessions may be canceled due to inclement weather, therapist illness, or other reasons beyond the therapist's control. In these cases, the therapist will work with you to reschedule.
- For 1-on-1 sessions, a guardian or caregiver must remain accessible (within the premises or with the child during the session).
- Clients arriving late may not be guaranteed their full scheduled session time, though full payment will still be required.
Health & Safety
- Please notify your therapist of any medical conditions, injuries, or recent changes in health status.
- Sick policy: If you or your child are experiencing fever, vomiting, or contagious illness, please cancel the session (standard cancellation policies still apply).
Communication
- Email and/or phone communication will be used for scheduling, reminders, and updates.
- Please allow 24–48 hours for responses during business days.
Consent & Privacy
- All services require signed consent forms prior to participation.
- Function & Rhythm follows HIPAA guidelines to protect client privacy and confidentiality.
Pricing & Insurance
- At this time, Function and Rhythm Therapy is not partnered with insurance providers. All services are private pay.
- If you’d like, I can provide an invoice (often called a “superbill”) that you may submit to your insurance company for possible out-of-network reimbursement, however, services must still be paid at the time of service. Coverage varies, so it is always recommend to check directly with your provider to learn more about your options.
